Social Services Director SSD

You must hold a degree/licensing in the field of Social Services to be considered for this position.

Cupertino Healthcare and Wellness Center area is seeking a Social Services Director to join our care team.

Purpose: To ensure that the physical, mental, and emotional needs of our residents are met in the realm of Social Services.

Duties and Responsibilities

• Ensure the needs of our long-term care residents are met
• Maintain a community resource list
• Complete initial assessments
• Implement care plans
• Assist with discharge planning
• Document patient progress
• Coordinate family meetings
• Ensure regulatory compliance
• Other duties as assigned to meet the purpose of this role

Requirements and Qualifications

• A bachelor's degree in social work or a closely related field
• A master's degree a plus
• State licensure preferred (CSW, ACSW, LSW, LCSW)
• Experience with social work, preferably working directly with patients and their family members at a skilled nursing facility
• Bilingual communication skills a plus
• Maintains professional working relationships with all associates, vendors, etc.
• Maintains confidentiality of all proprietary and/or confidential information
• Understand and follow company policies including harassment and compliance procedures
• Displays integrity and professionalism by adhering to Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training

Physical Capacities: (With or Without the Aid of Mechanical Devices)

• Must be able to move intermittently throughout the workday.
• Possible bending, squatting, twisting and reaching
• Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
• Must meet the general health requirements set forth by the policies of this facility
• Must be able to stand for and walk for extended periods of time
• Must be able to cope with the mental and emotional stress of the position

Environmental Conditions: Inside work, normal temperatures, some noise, occasional fumes/odors, chemical exposure and minimal potential exposure to blood-born pathogens.

• SSD: 1 year (Required)
• Nursing home: 1 year (Preferred)

License/Certification:

• Social Work License (Required)
