We are currently seeking a Social Worker for Skilled Home Health Care to work in the San Francisco and Marin County areas. You should possess a demonstrated ability to work effectively with patients/families dealing with life-threatening illness and death, along with a strong working knowledge of social work practices.

Essential Duties & Responsibilities include (but not limited to):

Evaluates the need for medical social casework services to patients in their place of residence.
Communicates patient needs to the physician and home health care team.
Plans quality medical social work services.
Implements social work services to patient in their place of residence.
Evaluates medical social work services plan on an on-going basis.
Maintains assessment and progress documentation accurately.
Participates in the quality improvement program.

Education/Experience:

Master Degree in Social Work from a school accredited by the Council on Social Work Education.
Minimum one year social work experience in a health care setting.
Previous experience in a licensed home health agency preferred.

Certificates, Licenses, Registrations:

Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

Pay range:
$37.00 - $50.00 hourly
